SCHEDULE

• Dates of Need:
• Shift Times: onsite 7am-5pm, pager call (30 min call radius) from 5pm - 7am
• Provider Min # Shifts Required: Shifts listed below, will prioritize providers who are available for consistent coverage multiple shifts per month for continuity of care.
• Guaranteed Hours per Day: 10
• Hours Worked for OT to apply: 10 hours per day
• Rates to be Filled Out on Bid: Clinic and Call: Regular, Orientation, Overtime, Holiday, Night Call, 24 Hour Call, Call-Back.

ADDITIONAL DETAILS
We will prioritize providers who are available for consistent coverage multiple shifts per month for continuity of care. Below shifts are needed, 7a-5p with overnight beeper call post shift.
Open shifts due to a late cancellation:

November

• November 6-8
• November 16-18

December

• December 4-6
• December 21-23

We will also have ongoing shifts in 2027 but are needing to prioritize providers who can assist with the above shifts first. Confirmed providers will be added to the pool for ongoing 2027 coverage. The site estimates we will need one week day 3 day coverage and one Fri-sat-sun per month on and long term and consistent basis.
