Own continuity of care for patients needing more than virtual outpatient therapy. Work remotely from anywhere in the US as a full-time W2 team member. Key Responsibilities: Identify high-acuity patients and assess their needs, preferences, and coverage. Build individualized transition-of-care plans and connect patients to appropriate services. Execute referrals, schedule appointments, transmit records, and confirm attendance. What We're Looking For: Calm, relentless problem-solvers with 2+ years of healthcare case management experience.
