Job Summary
Capital Hospice & Palliative Care is seeking an experienced Clinical Manager to oversee clinical staff, coordinate patient care, and ensure the delivery of high-quality hospice services. The Clinical Manager will support clinical operations, supervise nursing personnel, and collaborate with the Clinical Director and interdisciplinary team.

Key Responsibilities
- Review patient referrals, assess care needs, and coordinate appropriate hospice services.
- Supervise, guide, and support clinical personnel to ensure quality patient care.
- Review patient diagnoses, prognoses, medications, and clinical progress.
- Assist clinicians with care planning, treatment goals, and symptom management.
- Participate in interdisciplinary team meetings and coordinate patient care.
- Conduct quarterly medical record reviews and communicate recommendations.
- Assist with recruiting, interviewing, onboarding, and orienting clinical staff.
- Support staff education, training, and continuing development.
- Participate in quality assurance and performance improvement activities.
- Support hospice policies, regulatory compliance, and organizational goals.
- Participate in community outreach and activities that promote hospice services.
- Perform other duties as assigned by the Clinical Director.
Qualifications
- Current, active Registered Nurse (RN) license in Illinois.
- BSN from an accredited nursing program preferred.
- Hospice or home health care experience required.
- At least 2 years of management or supervisory experience .
- Experience working with interdisciplinary healthcare teams.
- Strong clinical judgment, leadership, and communication skills.
- Knowledge of hospice philosophy, end-of-life care, and pain and symptom management.
- Current CPR certification.
- Valid driver's license, reliable insured vehicle, and ability to travel as needed.
