Overview

We are seeking clinicians to perform in-home health and wellness assessments for patients in Arizona. Work is scheduled based on provider availability and county needs.

Schedule

Monday - Sunday availability window

7:00 AM - 7:00 PM availability window (per Order description)

5 to 8 assessments per day noted in the Order description

Responsibilities

Conduct in-home health and wellness assessments including history and physical evaluations

Review medications and dosages

Perform basic screenings as applicable (per Order description)

Complete documentation using an iPad-based system (per Order description)

Requirements

Candidates must have at least two years of clinical experience within the past five years to qualify.

Active, unrestricted Arizona license as an MD, NP, or PA (per Order description)

National certification required for NP or PA (per Order description)

CAQH profile and NPI required (per Order description)

Auto insurance required (per Order description)

Must be available at least one weekday per week (weekend-only availability is not accepted, per Order description)

Travel radius requirement: minimum 75 miles from provider home address (per Order description)

Travel and Lodging

Travel and lodging may be coordinated and paid directly by the client based on assignment expectations (per Order description)

Credentialing is handled by the client (per Order description)
