• Lead a collaborative care team that shares responsibility for outcomes
  
• Mentor and supervise APPs while maintaining clinical oversight
  

• Keeping patients healthier and out of the hospital
  
• Delivering an excellent patient experience (CG-CAHPS)
  
• Closing preventive and chronic care gaps
  
• Managing total cost of care responsibly

• MD or DO
  
• Board Certified or Board Eligible (IM, FM, or Geriatrics)

• $250,000 - $306,714 base salary (experience-based)
