Physician - Primary Care / Post-Acute Care (Sign-On / Relocation Bonus - Negotiable)

Schedule: Monday-Friday Round-and-Go Model

Position Overview: We are seeking a dedicated Primary Care Physician (MD/DO) to provide comprehensive care to patients in skilled nursing, assisted living, memory care, and post-acute settings. This role offers a flexible round-and-go schedule, clinical independence, and opportunities for additional income through Medical Directorships.

Key Responsibilities

•Provide primary care to geriatric and post-acute patients.

•Manage acute and chronic medical conditions.

•Conduct examinations, manage medications, and order labs/imaging.

•Coordinate care with nurses, NPs, PAs, and other healthcare professionals.

•Visit assigned facilities approximately twice per week.

•Participate in limited rotation-based call coverage.

•Opportunity to serve as Medical Director for assigned facilities.

Qualifications:

•MD or DO with training in Family Medicine, Internal Medicine, or Geriatrics.

•Active or eligible MA Medical license.

•Active DEA license.

•Skilled nursing, post-acute, or geriatric experience preferred.

•New graduates with geriatrics or post-acute exposure may be considered.

•Ability to work independently and collaboratively.
