• Position : Family Medicine or Adult Medicine opportunity
• Type: 100% Outpatient
• Call: 1:5 telephone only, and supported by nurse advisory line
• Model: Patient Center Medical Home Model
• Start date: 60-90 Days
• Schedule: Mon - Fri with flexible scheduling
• Job Description: Typical outpatient Primary Care services
• Patient Population: All Ages or Adult Medicine
• Patient Load: 18-22 PPD
• EMR : Epic
• Support: 2 experienced APPs Full admin staff
