About the Opportunity
A well-established medical group on Long Island is seeking an experienced Nurse Practitioner or Physician Assistant for an evening shift position at a skilled nursing facility in Bayshore, NY. This W-2 role suits an APP who is confident in long-term care medicine, wants a consistent PM schedule, and enjoys working closely with facility staff to deliver high-quality patient care.

Position Highlights

  • Shift: PM / Evening, 4:00 PM - 10:00 or 11:00 PM
  • Compensation: $160,000 - $170,000 annually
  • Employment Type: W-2
  • Setting: Skilled Nursing Facility (SNF)
  • Location: Bay Shore, NY (Suffolk County, Long Island)
  • Patient Volume: About 25 patients per shift

Responsibilities

  • Manage a census of about 25 SNF patients per shift
  • Complete progress notes and documentation
  • Evaluate and treat acute changes in condition during evening hours
  • Coordinate care with nursing, rehab, and facility staff
  • Collaborate with the supervising physician on care plans

Requirements

  • Prior SNF / long-term care experience is required
  • Active NY NP or PA license in good standing
  • Active DEA registration
  • Comfortable managing a high-volume census independently during evening hours
