TinkBird Healthcare Staffing is seeking a Nurse Practitioner or Physician Assistant for a locum tenens opportunity supporting a community-based PACE program in Lexington, North Carolina. This position provides primary care to a geriatric and underserved patient population within a collaborative, interdisciplinary care model.
Position Details
Job Type: Locum Tenens

Setting: Outpatient PACE Clinic
Start Date: ASAP
Assignment Length: 2 months initially, with potential for extension
Extension: Potential to extend on a month-to-month basis
Schedule: 4 days per week
Hours: 8:30 AM 4:30 PM
Call: None
Weekends: None
Holidays: No holiday coverage
EMR: Greenway
Location: Lexington, North Carolina
Local candidates preferred
Practice Environment
Geriatric and underserved patient population
Community-based PACE program
Collaborative, interdisciplinary team-based care model
Coordinated referrals to specialists
In-person patient care only; no telehealth
Clinical Responsibilities
Provide comprehensive primary care to geriatric patients
Evaluate, diagnose, and manage acute and chronic medical conditions
Provide ongoing care within a multidisciplinary PACE model
Coordinate care and referrals with specialists and other members of the care team
Support the health and wellness needs of an underserved patient population
Qualifications & Requirements
Nurse Practitioner or Physician Assistant
Board Certified
Minimum of 2 years of experience in Primary Care, Hospice, or Long-Term Care
BLS/CPR and ACLS required
Active North Carolina license or ability to obtain
Must be eligible for Medicare/Medicaid enrollment
Local candidates preferred
Provider must have the availability to commit to the initial 2-month assignment and potential month-to-month extensions
Holiday Schedule
The following holidays are observed:
November 26, 2026: Thanksgiving
December 25, 2026: Christmas
January 1, 2027: New Year's Day
