A growing, physician-led post-acute care organization is seeking a dedicated Physician to provide primary care services within long-term care communities across Pennsylvania.

Clinical Responsibilities

• Conduct admissions and new patient evaluations
• Manage chronic and acute medical conditions in the LTC setting
• Perform routine follow-ups and regulatory visits
• Collaborate with NPs, nursing staff, therapists, and facility leadership
• Communicate with families and caregivers
• Document encounters using a company-issued mobile device

The focus is on quality, coordination, and patient-centered care within a structured post-acute environment.

Schedule

• Monday Friday
• Daytime hours
• Minimal on-call rotation
• No regular nights or weekend rounding
