Kansas Licensed Patient Care Coordinator

To support infusion services, the part-time Kansas Licensed Patient Care Coordinator will manage patient intake, coordinate care with healthcare providers, and ensure timely communication and documentation while working remotely. Key responsibilities: Receives and executes the initial patient intake process to ensure timely scheduling of nursing services Maintains communication with healthcare personnel and documents care coordination accurately Tracks patient visit schedules and assists with staffing for home health agencies Required qualifications: 6+ months of customer service or healthcare-related experience Intermediate level of computer proficiency, including Microsoft Office and web applications Must be 18 years of age or older
