Responsibilities

• Conduct comprehensive assessments of residents' needs and develop individualized care plans in accordance with the Minimum Data Set (MDS) guidelines.

• Collaborate with interdisciplinary teams to ensure seamless care coordination and delivery of high-quality services.

• Monitor and report resident progress and concerns to family members, care providers, and other stakeholders.

• Participate in quality improvement initiatives to enhance patient outcomes and drive continuous improvement.

• Maintain accurate and detailed records of assessments, care plans, and resident interactions.
