Inglemoor Rehabilitation and Care Center, a leader in the skilled nursing industry, is seeking a dedicated and experienced MDS Registered Nurse to join our healthcare team on a Full Time basis in Livingston, New Jersey .

About the Role

As a key member of our interdisciplinary team, the MDS Coordinator will play a crucial role in ensuring the highest level of care for our residents. The ideal candidate will have at least 3 years of experience in MDS and long-term care regulations, with a strong background in healthcare and a passion for delivering exceptional patient care.

Responsibilities

  • Coordinate and maintain the Minimum Data Set (MDS) process for all long-term residents, ensuring accurate and timely assessments
  • Develop and implement care plans that meet a resident-centered plan of care, in collaboration with the interdisciplinary team
  • Conduct MDS assessments, evaluations, and progress notes, ensuring compliance with regulatory requirements
  • Provide education and training to staff on MDS process and documentation requirements
  • Utilize EMR and HER systems effectively for documentation and reporting purposes
  • Familiar with EPIC, PCC, and EMR/EHR, CMS 5-star
  • Strong analytical skills with attention to detail for documentation review and coding accuracy
  • Ability to work independently as well as part of IDCP team
  • Excellent communication skills
