Galileo Health is seeking a culturally competent Nurse Practitioner to join our Population Health team. This role uses phone and video-based care to provide high-quality follow-up care to post-hospital discharge patients.

You’ll conduct telephonic outreach, coordinate care across hospital and community providers, and collaborate with our RN and administrative teams. A SC license and 5+ years in primary care or geriatrics are preferred, with weekend coverage as needed.
