Qualifications

• MD or DO from an accredited medical school
• Completion of an accredited ENT residency program
• American Board Eligible or Board Certified in Otolaryngology
• Ability to obtain an unrestricted Pennsylvania medical license
• Ability to obtain required credentials
• DEA certification required
