• 18 - 25 patients per day depending on SNF census
• Skilled nursing facility (SNF) setting adults only
• Open to internal medicine or family medicine providers
• Regulatory visits required every other 60-day visit
• Hospital privileges required
• 6-month locums engagement with potential for permanent placement
