  • Conventional Radiotherapy
  • External Beam Radiation - IMRT
  • Stereotactic Body Radiation Therapy (SBRT)Submission Requirements:To expedite the credentialing process, the following documents must be available at the time of acceptance:
    • Board Certification: Required
    • Ohio State License: Preferred; providers willing to obtain a license are accepted
    • BLS/ACLS Certification: Required
    • Availability: Full-time availability only
    • Flu Vaccine: Required at the time of credentialing
