- Valid IA license - REQUIRED
- Hold current and unrestricted state and federal authority to prescribe and administer medications as necessary or appropriate to the field of practice- REQUIRED
- DEA - REQUIRED
- BLS Certification and maintain recertification - REQUIRED
- Clean malpractice and license history HIGHLY PREFERRED / PREFERRED
- Does the provider require visa support H-1B cap exempt (Note in Additional Submission Details) REQUIRED
- Submit Self-Query NPDB pulled within 30 days from submission at time of presentation- REQUIRED
