Remote Behavioral Health Navigator & Care Coordinator

University Hospitals Pain Management seeks to strengthen population health outreach within Behavioral Health Ambulatory services. You will analyze data to identify at‑risk patients, educate families, and connect patients to UHACO and community resources to address barriers to care.

Role involves scheduling BH appointments, documenting outreach, and coordinating with providers to guide patients through diagnostics, specialty services, and community referrals, while maintaining PHI compliance.
