Remote Cardiac Care Navigator for CHF Support

Seamlessassist is seeking a Remote Care Navigator for Cardiac care coordination. This full-time, 40-hour/week role focuses on telephonic outreach, care plan support, CMS-compliant documentation, and coordination with the care team.

Supervised by RN Care Managers, the position aims to reduce hospitalizations and enhance patient self-management. The role requires active MA certification or equivalent, at least 2 years in care coordination, and strong English communication.
