Remote Cardiology Pharmacist for GDMT & Value-Based Care

Seamless Assist is seeking a remote, full-time Clinical Pharmacist with cardiology expertise to support value-based care programs. This role focuses on GDMT optimization, titration, and patient education, functioning as a clinical partner within a nurse-led care team.

You will collaborate with RN Care Managers and cardiology providers, review therapy, and document recommendations in the EMR to improve outcomes for high-risk cardiac patients.
