Remote Care Transition Coach

Molina Healthcare is seeking a care transition coordinator to support member discharge from hospital to home and other settings. This predominately work-from-home role emphasizes reducing readmissions and coordinating with hospitals, providers, and families.

You will conduct face-to-face visits when needed, educate members on medication management and follow-up care, and collaborate with an interdisciplinary team while using CTI or similar models.
