Remote Patient Care & Programs Coordinator

somewhere is seeking a Patient Care & Program Coordinator to own the 12-month patient journey in a remote, part-time capacity. This non-clinical role coordinates appointments, lab kits, and adherence to physician protocols, while maintaining patient confidentiality and high-touch service.

You'll manage 5–20+ active monthly patients, track PROs, and act as the administrative bridge between patients and functional medicine physicians.
