Remote Pharmacist: Telehealth MTM & Care Coordination

Your Health is seeking a Pharmacist to serve patients across the Grandstrand area via virtual visits from home. This full-time, salary-based role offers a monthly bonus and a Monday–Friday 8:00am–5:00pm schedule.

You will collaborate with care teams to optimize medication therapy, provide MTM, and help reduce rehospitalizations while delivering compassionate, patient-centered care. The position supports primary care services across South Carolina and Georgia, with opportunities to advance in a
