Remote Physician Collaborator for Senior Primary Care

Humana's CenterWell Senior Primary Care is seeking a Physician Collaborator for a fully remote role. You will collaborate with APPs delivering wraparound services to fracture patients who cannot see their PCP in person. Remote work with ~5 hours monthly, reporting to the Associate Director.

Must hold unrestricted MD/DO license in eligible states and be board certified or eligible, with 2+ years of collaboration experience with mid-level providers. Travel may be occasional.
