Remote Physician Collaborator - Senior Primary Care

Humana Inc. is seeking a Physician Collaborator to join CenterWell Senior Primary Care in a fully remote role. You will report to the Associate Director and support patients who have fractures in the last 6 months, collaborating with APPs delivering wraparound services.

Requires MD/DO degree, unrestricted medical license in KY, TN, MO, NC, TX, IN, board certification or eligibility, and 2+ years collaborating with mid-levels. About travel and flexible schedules may apply.
