Remote Post-Acute Care Manager RN (Telephonic)

CenterWell Home Health, a Humana company, is seeking a Telephonic Registered Nurse Care Manager to support patients transitioning in and out of services. This remote role focuses on determining appropriate post-acute care options and coordinating with the clinical team to ensure continuity of care.

You will work a 40-hour week on a rotating schedule with 10-hour shifts, may travel for training, and must hold an RN license with an Associates Degree.
