Remote RN Care Manager - Complex Care & Coordination

Prominence Health seeks an RN Care Manager located in Texas to provide care management for medically and socially complex members. You will develop personalized care plans, coach for adherence, and coordinate with contracted physicians and community resources to improve outcomes.

Under CMSA Standards of Practice, you will participate in SNP CM program activities and ensure model of care meets regulatory requirements. Remote in TX and strong English/Spanish communication are favored.
