Remote RN: Integrated Care Manager for Complex Needs

Sentara Health is seeking an Integrated Case Manager, RN to support Eastern Shore members with a focus on behavioral health, waiver services, and complex care management. The role is remote with requirement to reside on Virginia’s Eastern Shore and travel 50–75% for in-home visits.

The position requires an RN license, an Associate degree as minimum, and at least three years of clinical nursing. BSN preferred. Competitive pay and full benefits are provided.
