Remote Telephonic Care Coach - Wellness Support

Humana Inc. is seeking a Telephonic Care Coach to support members’ wellness by coordinating services and resources. The role reports to the Manager, Care Management and requires 2+ years of experience, strong communication skills, and comfort with electronic health information systems.

The position is remote with occasional travel to Humana offices for training. Standard hours are 9am-5:30pm EST, 40 hours per week, with potential scheduling variations based on business needs.
