Position Overview

We are currently seeking a dedicated and compassionate Speech-Language Pathologist (SLP) to join our rehabilitation team.

Duties & Responsibilities

The Speech-Language Pathologist will be responsible for:

  • Assessing referred patients who require rehabilitation services.
  • Providing direct patient care by evaluating medical conditions, functional capabilities, limitations, restrictions, and potential for rehabilitation.
  • Developing and administering individualized treatment programs with specific goals based on each patient's abilities, needs, and tolerance, under the direction of the physician.
  • Establishing realistic, measurable, and achievable goals for patients.
  • Documenting and maintaining accurate records of patient conditions, progress, and treatment.
  • Educating patients and their families regarding appropriate therapy techniques and strategies.
  • Completing evaluations, weekly progress notes, recertification documentation, and discharge summaries.
  • Collaborating with nurses, physicians, therapists, and other healthcare professionals to provide comprehensive, patient-centered care.
  • Performing other duties and responsibilities as assigned by the Director of Rehabilitation.
  • Maintaining compliance with confidentiality requirements, HIPAA regulations, and Federal Residents' Rights.

Qualifications & Requirements

  • Graduate of an accredited Speech-Language Pathology program at a recognized college or university.
  • Current and valid Speech-Language Pathology license and registration in the state of practice.
  • Experience in long-term care and/or geriatric care is preferred but not required.
  • Strong communication, interpersonal, organizational, and clinical skills.
  • Commitment to providing compassionate, resident-centered care.
