Requirements:
- Board Eligible (within 5 years of residency) or Board Certified in Urology - REQUIRED

- Most recent NPDB Self Query Report (within 30 days active) - REQUIRED no exemption, required at presentation

- DEA (must be obtained prior to start) - (note in "Additional Submission Details" field) - REQUIRED no exemptions
